Make a handwritten or a computer spreadsheet of your medications, vitamins, pain relief, and supplements including Px name, dose, times, color, Px # and pharmacy name, address and phone number. Having this printed out and available when you are seen by a nurse prior to a doctor's appointment will make them happy to see you so organized and prepared.

Regular Check-ups
Set-up a medical folder or three ring binder of all your medical documents. Be sure to sign paperwork at all of your providers to access your medical records. Ask for lab reports, visit summaries, x-rays, CD’s etc. Take this collection with you to every appointment as back-up. You and/or your partner will be glad you did at some point where there is some confusion or issue. As above they will appreciate your being organized.
